All Service user/Patient details will remain confidential and will only be used for administrative purposes to help us assist with your enquiry. Any information you provide is encrypted and submitted over a secure connection and cannot be viewed by anyone else on the web.

REFERRAL DETAILS

Referral process

A unique reference number is generated for each enquiry which ensures subsequent communication between Comfort Care Services, the referrer and patient, is efficient, accurate and safe. To ensure you successfully receive electronic correspondence from us, please add enquiries@comfortcareservices.com to your safe list or address book.

1. Complete the online referral form, including the patient’s contact details
2. Our case manager will contact the patient directly and confirm costs and location of appointment. We will advise that our team will be in touch to arrange the most convenient appointment with the appropriate Comfort Care Services specialist for that patient
3. The appointment is then confirmed directly between the patient and Comfort Care Services
4. REFERRAL will receive confirmation from us when the time, date, location and Comfort Care Services involved has been agreed with the patient
